Research on relationship among transition shock,adversity quotient and feedback seeking behavior of newly graduated nurses

Objective To investigate the influence of transition shock and adversity quotient on the feedback seeking behavior of newly graduated nurses,and to clarify the model of the relationship among them.Methods The general information questionnaire,adversity response profile,the transition shock of new graduated nurse scale and feedback seeking behavior of newly graduated nurse scale were used to investigate 216 newly graduated nurses from 2 hospitals in Jiangsu Province.Results Transition shock of new graduated nurses had a negative impact on feedback seeking behavior(r=-0.707;P<0.01),and the transition shock could affect the feedback seeking behavior through the mediating influence of newly graduated nurses’ adversity quotient,with a mediating effect of 56.31%.Conclusion Adversity quotient is a mediating variable between the transition shock and feedback seeking behavior of newly graduated nurses. Nursing managers and educators should help newly graduated nurses realize their self-potential and enhance feedback-seeking behaviors.

Application of calf circumference in evaluating decreased skeletal muscle mass in maintenance hemodialysis patients

Objective To explore the relationship between calf circumference and skeletal muscle index(SMI),handgrip strength,and usual gait speed in maintenance hemodialysis(MHD) patients,and to investigate the effect of calf circumference in the evaluation and diagnosis of sarcopenia. Methods A cross-sectional study was conducted in 613 MHD patients(379 men and 234 women)from 2 tertiary hospitals in Lianyungang city and Xuzhou city,Jiangsu Province by the convenience sampling method. The SMI of the patients was measured by bioelectrical impedance analysis,and the indicators such as handgrip strength,usual gait speed and calf circumference were measured. The correlation between calf circumference and SMI,handgrip strength,and usual gait speed was analyzed. The area under the receiver operating characteristic curves(AUC) was used to test the accuracy of calf circumference to predict decreased skeletal muscle mass(DSMM),low muscle strength,low physical performance and to diagnose sarcopenia. Results The calf circumference of MHD patients was positively correlated with SMI,handgrip strength,and usual gait speed. The optimal cut-off values of calf circumference for predicting DSMM were 32.7 cm(with the AUC of 0.909,the sensitivity of 0.889,the specificity of 0.799) in men and 30.2 cm(with the AUC of 0.837,the sensitivity of 0.744,the specificity of 0.808) in women. The optimal cut-off values of calf circumference for predicting sarcopenia were 32.7 cm(with the AUC of 0.889,the sensitivity of 0.907,the specificity of 0.745)in men and 29.7 cm (with the AUC of 0.844,the sensitivity of 0.766,the specificity of 0.797) in women. Conclusion Calf circumference is positively correlated with SMI,handgrip strength,and usual gait speed. When the calf circumference is less than 32.7 cm in men and 29.7 cm in women,it indicates that the patients are very likely to suffer from DSMM and sarcopenia.

Establishment and enlightenment of a predictive model for decreased skeletal muscle mass in maintenance hemodialysis patients

Objective To construct a predictive model of decreased skeletal muscle mass in maintenance hemodialysis patients and to apply it in clinical practice to evaluate the effects of the model. Methods A total of 304 maintenance hemodialysis patients in a tertiary hospital in Lianyungang were selected as the training set by convenience sampling method,and various indicators of the decreased skeletal muscle mass group(n=74) and the non-decreased skeletal muscle mass group(n=230) were analyzed by univariate analysis. Logistic regression analysis was used to construct the prediction model and the model was simplified it. The area under the ROC curve was used to test the model effects,and 309 maintenance hemodialysis patients in a tertiary hospital in Xuzhou were selected to externally verity the effects of the model. Results The study finally included 4 factors,namely gender,BMI,calf circumference and predialysis creatinine,to construct the risk prediction model. The area under the ROC curve of this model was 0.911;the area under the ROC curve of the simplified model was 0.911;the accuracy rate of practical application was 75.40%. Conclusion The risk prediction model constructed in this study can predict the risk of decreased skeletal muscle mass in patients undergoing maintenance hemodialysis,and all the indicators are simple and easy to obtain,thus providing references for medical staff to carry out early indenfication and take preventive care interventions for high-risk patients in the early stage.

Construction of evaluation index system for core competence of clinical nutrition specialist nurses

Objective To establish an evaluation index system of core competence for clinical nutrition specialist nurses. Methods The first draft of the evaluation index system of core competence for clinical nutrition specialist nurses was prepared by referring to domestic and foreign literature and research group discussions. 18 experts were consulted with 2 rounds of Delphi consultations to complete the screening and modification of indicators and to determine the weights. Results The evaluation index system of core competence for clinical nutrition specialist nurses was formed,including 5 first-level indicators(clinical nutrition professional ability,leadership and management ability,professional humanistic traits,guidance and collaboration ability,professional development ability),14 second-level indicators,and 72 third-level indicators. The effective response rates of 2 rounds of expert consultations were both 100%,and the expert authority coefficients were both 0.910. The Kendall coordination coefficients of the first,second and third level indicators in the second round were 0.602,0.558,0.347(P<0.001). Conclusion The evaluation index system of core competence of clinical nutrition specialist nurses is scientific,reasonable,comprehensive and targeted,which can provide references for the training and evaluation of clinical nutrition specialist nurses.
